COMMUNITY ACCESS PROGRAM (CAP) MANAGER POSITION - MAX HIGBEE CENTER
Manager will provide direct recreation and community access services to people 14 years of age and older with developmental disabilities. The position requires flexibility and strong leadership skills, as managers will supervise staff, as well as lead groups of various sizes independently. Recreation activities include BINGO, art projects, outdoor games, karaoke, YMCA workouts, library visits, music programs and much more. Manager must show respect for all people and maintain strong personal boundaries. In addition to working directly with people with developmental disabilities, this position includes jobs duties that maintain the overall functioning of the organization. CAP Manager is responsible for program development, fiscal management and bookkeeping, and personnel administration for Max Higbee Center's Community Access Program (CAP). DISCHARGE REFERRAL COORDINATOR
WAHA partners with Northwest Regional Council and PeaceHealth St. Joseph Medical Center to assist patients with their transitions from the hospital to home or other care settings. The goal is to improve the quality of care for the patients, while also reducing health care costs by reducing the likelihood that the patient will return to the hospital unnecessarily.
We are seeking to fill multiple, part-time positions that can range from 24-32 hours per week or 5-10 hour per week contractual positions.
The discharge referral coordinator is an employee of WAHA stationed at PeaceHealth St. Joseph Medical Center (or potentially other hospitals), with a matrixed relationship with the hospital discharge department. This individual will oversee the day-to-day operations of referrals of eligible beneficiaries within the context of the hospital's and the community's broader referral framework.
